Type specimen: SUI 104381. Its type locality is Martin roadcut, 2.5-4.4 m above base, which is in a Moscovian marine shale in the Four Corners Formation of Kentucky.

Distribution: found only at Martin roadcut, 2.5-4.4 m above base
